June 10th, 2018 - FOR IMMEDIATE RELEASE
Moondown Pharmaceuticals is voluntarily recalling lot WXT667 of Milkflower Root Injection, USP, 0.9 mg/mL, 1 mL in 2.5 mL, single-use cartridge syringe system to the hospital/institution level due to the potential presence of toxic waste matter inside the syringe cartidges. In the event that impacted product is administered to a patient, the patient has an extremely high likelihood of experiencing adverse events ranging from local irritation, allergic reactions, phlebitis, end-organ granuloma, tissue ischemia, pulmonary emboli, pulmonary dysfunction, pulmonary infarction, septic shock, early death - and extreme toxicity. The risk is reduced by the possibility of detection, as the labeling contains a clear statement directing visual inspection of the product for particulate matter, toxins, discoloration, and presence of GMOs prior to administration. To date, Moondown Pharmaceuticals LLC has received no reports of any adverse events associated with this issue for this lot.
